Eur Radiol:高分辨率PCD-CT发现CTEPH患者肺微血管病变影像学特征

本研究表明,CTEPH患者低灌注区域内肺小血管病变征象显著多于高灌注区,而随肺动脉压升高,病变表现逐渐趋于均质化,提示病变波及范围扩大,局部灌注差异减弱。

近日,一项基于29例慢性血栓栓塞性肺动脉高压(Chronic Thromboembolic Pulmonary Hypertension, CTEPH)患者的前瞻性研究证实,**光子计数探测器CT(Photon-Counting Detector CT, PCD-CT)**凭借超高空间分辨率,能够有效识别CTEPH患者肺内微血管病变的细微影像学表现,为后续治疗决策提供重要依据。这一成果进一步扩展了影像学在肺动脉高压微血管病理机制评估中的应用价值,对复杂小血管病变的识别具有重要临床意义。

研究背景

慢性血栓栓塞性肺动脉高压是因肺动脉内血栓机化并纤维化阻塞血管腔导致的致死性肺血管疾病。即便部分患者通过肺动脉内膜剥脱术(Pulmonary Endarterectomy, PEA)成功解除大血管阻塞,仍有部分病人术后残余或复发性肺动脉高压,其核心机制即肺小血管病变(microvasculopathy),但目前常规影像学手段难以直观识别。近年来,PCD-CT因其优异的空间分辨率及组织对比,逐渐被应用于肺血管病领域,成为观察远端微血管病变的潜力工具。

研究设计

本研究纳入29例确诊CTEPH患者,均接受高分辨率PCD-CT检查。根据肺灌注马赛克征(mosaic perfusion)表现,从每位患者影像中选取最多3对高灌注(hyperattenuating)与低灌注(hypoattenuating)区域进行配对分析,共计86对区域。

影像学评估指标包括:

  • 模糊结节(ill-defined micronodules)

  • 小叶型磨玻璃影(lobular ground-glass opacities)

  • 弥漫性毛糙感(haziness)

  • 新生血管表现(neovascularity)

  • 胸膜下系统-肺动脉吻合(subpleural systemic-to-pulmonary anastomoses)

主要结果

  • 低灌注区(hypoattenuating areas)中肺微血管病变征象显著更多

    • 模糊结节(p = 0.008)

    • 小叶型磨玻璃影(p = 0.01)

    • 毛糙感(p = 0.003)

  • 新生血管化表现(neovascularity)两区域间无显著差异(p = 0.43)

中心型及周围型CTEPH患者亚组分析中:

  • 中心型+周围型患者(22例,66对配对区域)呈现相似趋势

  • 单纯周围型患者(7例,20对配对区域)高灌注区无微血管病变表现

根据肺动脉平均压分层分析:

  • mPAP ≤ 42 mmHg患者(45对):

    • 低灌注区模糊结节(p = 0.003)、毛糙感(p < 0.001)更常见

    • 胸膜下系统-肺动脉吻合显著更多见(p = 0.02)

  • mPAP > 42 mmHg患者(41对):

    • 高、低灌注区间微血管病变征象差异不显著,呈均质化表现

研究结论

本研究表明,CTEPH患者低灌注区域内肺小血管病变征象显著多于高灌注区,而随肺动脉压升高,病变表现逐渐趋于均质化,提示病变波及范围扩大,局部灌注差异减弱。PCD-CT可清晰描绘微血管病变特征,为CTEPH病理生理机制研究及临床治疗策略制定提供精准影像依据。

临床启示

① 肺微血管病变与CTEPH残余肺动脉高压密切相关

研究证实,部分CTEPH患者在成功行PEA术后仍有残余肺动脉高压,其中关键病理基础即远端肺小血管病变。PCD-CT首次在成像层面揭示CTEPH患者肺微血管病理特征,有助于术前评估及预后判断。

② PCD-CT有望成为CTEPH微血管病变影像学标准方法

传统CT、MRI及V/Q扫描等手段对微血管病变识别有限,而PCD-CT凭借超高分辨率及密度对比分辨优势,能够直观识别模糊结节、磨玻璃影等细微病变,为早期干预提供依据。

③ 疗效评估及个体化决策新工具

依据PCD-CT微血管病变表现,临床可辅助判断PEA术后复发风险,或指导是否联合肺动脉扩张、靶向药物、球囊肺动脉成形术(BPA)等个体化综合治疗方案。
