Blood:PD-1的免疫逃逸是富含T细胞/组织细胞的大B细胞淋巴瘤的定义特征

PD-1/PD-L1信号在肿瘤微环境中的空间分辨特征定义了富含T细胞/组织细胞的大B细胞淋巴瘤;3/5的复发性/难治性TCRLBCL患者对PD-1阻断剂(派姆单抗)单药治疗显示出客观临床缓解。

富含T细胞/组织细胞的大B细胞淋巴瘤(TCRLBCL)是弥散性大B细胞淋巴瘤(DLBCL)的一种侵略性变异类型,其特征在于强大而无效的浸润免疫细胞内存在罕见的恶性B细胞。TCRLBCL的免疫逃逸机制基础定义不清,目前还没有针对性的治疗措施。

Griffin等在多机构TCLRBCLs队列中对PD-1/PD-L1通路进行了遗传和定量空间分析,发现在64%的病例中,恶性B细胞携带与PD-L1表达上调相关的PD-L1/PD-L2拷贝数增加或扩增。

通过对肿瘤微环境中的多参数细胞表型数据进行定向和无监督的空间分析发现,TCRLBCL的特征为,在肿瘤免疫微环境中,恶性B细胞被异常高表达PD-L1的TAM和PD-1阳性T细胞包围。而且,具有空间分辨性的免疫特征的无偏倚聚集性可将TCRLBCL与相关的B细胞淋巴瘤亚型(包括经典霍奇金淋巴瘤和DLBCL-NOS)区别开来。

最后,研究人员观察到,5位复发性/难治性TCRLBCL患者中有3位经PD-1阻滞治疗获得临床缓解,包括2例完全缓解和1例部分缓解。

总而言之,本研究结果表明PD-1信号是TCRLBCL的一条免疫逃逸信号,同时还展示了空间分辨性免疫特征协助诊断分类和明确不同肿瘤类型采用免疫治疗的优先顺序的实用性。
