实施多学科心源性休克(CS)团队与改善CS生存率有关。

心源性休克,或称心因性休克,指因心室机能失常造成血液的异常流动,患者需立即送医。血液异常流动的症状有低血压、脉搏不齐或虚弱,此外还可能伴有呼吸困难、排尿过少、手脚发冷、神智不清、颈血管突出、皮肤湿腻等。心肌梗死是最常见的肇因。

美国国家心肌梗死注册数据库的研究表明,伴发心源性休克的急性心肌梗死患者中29%在接诊时即为休克,71%在住院期间发展为休克。此外,心源性休克也是急性心肌梗死患者的主要死因,并占到STEMI患者溶栓治疗后死亡患者死因的60%。

既往单中心研究表明,实施多学科心源性休克(CS)团队与改善CS生存率有关。为了全面描述有休克团队和没有休克团队的多个中心在管理CS方面的实践模式和结果。来自美国美斯达华盛顿医院中心心脏科和重症监护部的专家开展了相关研究,结果发表在JACC杂志上。

重症监护心脏病学试验网络是一个由北美的心脏重症监护病房(CICUs)组成的多中心网络。在每年2个月的收集期(2017-2019年;n=6872),采集每个CICU(n=24)的所有连续入院病人。使用反概率加权法比较了有和没有休克团队的中心的休克管理和CICU死亡率。

该研究纳入24个中心中的10个有休克小组。在1242例CS入院患者中,44%是在休克小组中心。各组在人口统计学、休克病因、序贯器官衰竭评估得分、终末器官功能障碍的生化标志物和侵入性血液动力学方面都很平衡。

总的来说,有休克团队的中心使用了更多的肺动脉导管(60% vs 49%;OR=1.86;95% CI:1.47-2.35;P < 0.001),整体机械循环支持(MCS)较少(35% vs 43%;OR=0.74;95%CI:0.59-0.95;P=0.016),以及更多高级类型的MCS(53% vs 43%;OR=1.73;95%CI:1.19-2.51;P=0.005)而不是主动脉内球囊泵。休克小组的存在与较低的CICU死亡率独立相关(23% vs 29%;OR=0.72;95% CI:0.55-0.94;P = 0.016)。

综上,在这项多中心观察性研究中,拥有休克团队的中心更有可能获得侵入性血液动力学,使用先进类型的MCS,并且风险调整后的死亡率更低。一个标准化的多学科休克团队方法可能会改善CS的结果。

深圳将投放5000台“救命神器”

市卫生计生委表示,该委已组织专家起草制定《深圳市“十三五”AED配置使用实施方案》,并设立财政专款,分年度公开招标采购5000台AED,逐步配置在全市各区有代表性的公共场所和人员密集场所。同时,按1∶10的比例为属地培训5万名初级救护员。
