甲钴胺如何使用?很多人都用错了!

甲钴胺又称甲基维生素B

甲钴胺又称甲基维生素B12,是维生素B12在体内的活性代谢产物。甲钴胺可促进神经细胞内核酸和蛋白质的合成,促进神经髓鞘和轴突再生,修复损伤的神经细胞,改善神经传导速度,广泛应用于临床周围神经病变等疾病的治疗。

【临床应用】

1.糖尿病周围神经病变
 
糖尿病神经病变是糖尿病最常见的慢性并发症之一,甲钴胺可明显改善糖尿病神经病变患者的临床症状、体征以及神经传导速度,并且安全性好,无明显不良反应。糖尿病神经病变诊治专家共识(2021年版)推荐:甲钴胺针剂500~1000 μg/d肌内注射或静脉滴注2~4周,其后给予甲钴胺片500 μg,每日3次口服,疗程至少3个月。
 
2. 巨红细胞性贫血

甲钴胺注射液用法:通常成人一次0.5mg,一日一次,一周3次,肌肉注射或者静脉注射。给药约两个月后,作为维持治疗每隔1-3个月可给予一次0.5mg。
 
3.二甲双胍所致维生素B12缺乏症
 
二甲双胍是治疗2型糖尿病首选药物,但是会对胃肠道产生一定的副作用。医生常用抑酸剂降低胃酸及内因子的分泌,而抑酸剂会减少维生素B12的吸收。若长期使用,患者可能会发生维生素B12缺乏症。甲钴胺可升高患者的血清维生素B12水平,降低同型半胱氨酸,减少四肢体感诱发电位的发生,是目前治疗由二甲双胍所致的维生素B12缺乏症的一种具有显著疗效的药物。
 
4.冠心病高同型半胱氨酸血症
 
甲钴胺是蛋氨酸合成酶的辅酶,将同型半胱氨酸转化为蛋氨酸,参与体内蛋氨酸与同型半胱氨酸的代谢。应用甲钴胺可降低冠心病伴高同型半胱氨酸血症患者的血浆同型半胱氨酸水平。
 
5.面神经炎
 
面神经炎是因茎乳孔内面神经非特异性炎症所致的周围性面瘫,临床表现为单侧周围性面瘫,可伴有同侧耳后疼痛或乳突压痛。甲钴胺可直接参与物质的甲基转换,促进核酸蛋白质及卵磷脂的合成,进而修复受损的神经纤维。有研究显示,甲钴胺能明显改善面神经炎患者的临床症状,促进面神经炎导致面瘫的神经功能恢复。
 
6.眼科疾病
 
甲钴胺已广泛应用于各种常见眼部疾病的治疗。据研究报道,甲钴胺可有效改善糖尿病视网膜患者的症状、降低血浆同型半胱氨酸血症水平,进而有效延缓糖尿病视网膜病变的进一步发展。此外,甲钴胺可有效治疗青光眼、中心性浆液性脉络膜视网膜病变、视神经损伤和眼肌麻痹等眼部疾病。
 
【不良反应】
 
用药30分钟内可出现严重不良反应,以皮肤系统损害、心血管系统损害和胃肠道不良反应为主,表现如下:
 
(1)皮肤系统损害:皮肤黏膜斑丘疹、面部红肿、过敏性荨麻疹、过敏反应;
 
(2)心血管系统损害:周围性水肿、低血压、晕厥;
 
(3)胃肠道反应:食欲不振、恶心、呕吐和腹泻。
 
【注意事项】
 
1.甲钴胺具有光敏性,在静脉用药调配过程中应尽量避光操作,如选择避光输液器,在条件允许的前提下调低室内光线(拉上窗帘、减少照明灯等),减少药品与光线的接触,进而保证药品质量和临床用药的安全性和有效性。
2.注射时:避免同一部位反复注射;避开瘢痕、硬结及发炎的地方;避开神经分布密集的部位,选择肌肉较厚,离大神经、大血管较远的部位。推药时,注意患者有无不适。
3.维生素B12不可替代甲钴胺。维生素B12(即氰钴胺)和甲钴胺都属于维生素B12家族,结构有所差异,因而临床应用和治疗效果也有所不同。甲钴胺主要应用于周围神经病变,维生素B12可应用于巨幼红细胞性贫血的治疗,也可作为神经炎的辅助用药。在治疗周围神经病变方面,甲钴胺的疗效和安全性优于维生素B12,因而维生素B12不可替代甲钴胺。
 
4.从事汞及其化合物的工作人员,不宜长期大量使用。甲钴胺与汞可形成甲基汞,损伤肾、脑和肺等器官。
